As a Assessment Specialist Supervisor, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities
- Directly supervises, manages, and oversees staff, including hiring, firing, and providing performance evaluations, salary recommendations, and individual development goals and objectives.
- Ensures functions of the department are in compliance with appropriate program requirements, CareStar policies and guidelines, and contract standards.
- Monitors intake inquiries and requests; ensures appropriate information and referral services are provided in a timely manner.
- Oversees the assignment of initial eligibility assessments and other clinical work to appropriate staff and new individuals to a Clinical Supervisor.
- Monitors and tracks applicant status until a final determination is made.
- Responds to inquiries and provides supporting documentation regarding cases as necessary.
- Prepares Appeal Summaries and participates in Appeal Hearings as a designee with applicable State agencies.
Minimum Qualifications
- Qualifications will vary from program to program, but may include: active license as a Licensed Social Worker, Licensed Independent Social Worker or Registered Nurse.
- Experience working with the populations served by the applicable program and experience in the home and community-based services environment.
- Ability to instruct, counsel, and evaluate subordinate professional staff; effective oral and written communication skills with attention to detail in assessment and in documentation.
- Adheres to the CareStar Rule in performance of job responsibilities.
- Understands and complies with CareStar Policies and Procedures.
- Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follows the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
