We are currently seeking an Assessment Specialist to join our Specialized Recovery Support Services program. Key Responsibilities
- Maintains and builds knowledge of the State and program requirements.
- Maintains and builds on knowledge of Medicare, third-party payers, community resources, and the Americans with Disabilities Act.
- Completes in-home visits to assess for eligibility for specified program and conducts safety assessments, in compliance with CareStar and department guidelines.
- Documents all applicant and case-related contacts and interventions in applicants record per agency and CareStar policy, including standards of content and timeliness.
- Provides assistance with community resource referrals as appropriate.
- Adhere to the CareStar Rule in performance of job responsibilities.
- Understand and comply with CareStar Policies and Procedures.
- Maintain confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
- Follow the Acceptable Use Policy while using any information systems owned or controlled by CareStar, Inc.
Minimum Qualifications
- Possesses a Bachelor's Degree in social work, counseling, psychology, or a related field; or Registered Nurse (RN), or Licensed Social Worker (LSW).
- Possesses a minimum of one-year post degree experience working with individuals with diagnosed chronic conditions and/or minimum of three years’ post degree experience working with individuals with severe and persistent mental illness.
- Knowledge of issues affecting people with severe and persistent mental illness and community-based interventions/resources for those individuals.
- Effective interpersonal (oral and written) skills to interact professionally and ethically with all age groups from a diverse population, including differing intellectual, cultural, ethnic, psychosocial and socio-economic backgrounds.
- Experience working with individuals with chronic conditions or severe and persistent mental illness
- Strong knowledge of Medicaid, behavioral health systems, and local community resources.
