As a Assessment Manager, you’ll work directly with individuals to assess their needs, develop personalized care plans, and connect them with essential services and supports. Key Responsibilities
- Monitors and evaluates performance of Team Members, including the development of processes and guidelines for the completion and review of in-person and telephonic assessments.
- Provides necessary coordination between activities, including assigning initial eligibility assessments to Team Members, to ensure outstanding service, accuracy, efficiency, and timely completion.
- Establishes parameters and expectations for productivity of Assessment Specialist and Assessment Specialist Supervisors. Implements performance improvement measures related to utilization and recommends personnel action as appropriate.
- Develops and manages processes for incoming calls to the agency from the general and professional public, including receiving applications and providing information and referral services.
Minimum Qualifications
- Current and active license as a Registered Nurse or Licensed Social Worker.
- Associate's Degree in Nursing or Bachelor's Degree in Social Work or related field required.
- Minimum of 5 years of program management or program analysis experience, preferably in a healthcare or social service setting; knowledge of Medicare, Medicaid, and community resources.
- Experience as a Manager/Supervisor in a health care or social service setting.
- Effective organization, interpersonal, communication and leadership skills.
- Effective computer skills, including familiarity with suite of Microsoft Office Programs.
- Reliable transportation to travel as business requires.
- Valid driver's license and car insurance as required by State law.
