Key Responsibilities
- Directly supervises, manages and oversees staff, including hiring, firing and providing performance evaluations, including and individual development goals and objectives.
- Participates in quality improvement activities, such as chart audits and project teams.
- Participates in hiring and orientating Case Managers and other professional staff as needed.
- Attends all meetings and training sessions as assigned.
- Participates as needed with formulating and conducting assigned staff training.
- Maintains open and direct contact with program staff personnel.
- Assists in preparation of program specific appeals.
- Enters information accurately into CareStar and/or program specific computer applications as required to perform job duties.
- Shares rotation of after-hours, on-call responsibilities.
- Serves on work groups, community groups, committees, or other task forces and attends meetings as requested.
• Provides direct clinical services to consumers as necessary to assist the clinical team.
• Educates, counsels, evaluates, and manages performance of the clinical team.
• Maintains/updates knowledge of Medicare, Medicaid, insurance and program specific
administered waivers.
• Maintains knowledge of the Americans with Disabilities Act.
• Coordinates assignment of the clinical team to consumers.
• Identifies problematic cases and refers them to the Clinical Manager
• Reviews the clinical team documentation per agency guidelines to ensure that
appropriate follow-up standards are met.
Minimum Qualifications
- Bachelor's degree in Social Work, Psychology, Sociology, Counseling, Gerontology or Nursing.
- At minimum one year of direct service experience with the elderly or persons with disabilities.
- Experience in completing assessments, service plan development and monitoring
- Completes all training curriculums and professional development requirements as specified by the assigned program.
• Possesses integrity, self-discipline, with the ability to work independently
within guidelines.
• Knowledge of Medicare, Medicaid, insurance and waiver programs and
community resources.
• Good problem-solving skills.

• Effective verbal and written communication skills with attention to detail in
assessment and in documentation.
• Ability to instruct, counsel and evaluate subordinate professional staff.
• Excellent organizational skills with the ability to prioritize projects and be attentive
to detail.

• Effective interpersonal skills with respect for others, including consumers, coworkers and other professionals within a diverse work environment.
• Demonstrates strong critical thinking and problem solving skills and flexibility
in relationship to job requirements.

•Computer skills sufficient to enter and process consumer information, report preparation, and use CareStar’s website and email, and familiarity with suite of Microsoft Office programs.
