We are currently seeking Home Visitor Nurse to join our Home Visitor program. As a Home Visitor Nurse, you’ll be responsible for working in a team environment of clinical and non-clinical Team Members to schedule, visit, and support families in the community using the evidence-based Family Connects Home Visiting Protocol. Key Responsibilities
Performs a health and physical assessment of the birthing parent and infant following the Family Connects home-visiting protocol. Provides a health and physical assessment of mother and baby in the home to determine well-being.
Assists parents in making appointments with healthcare providers, social services agencies, and other community referrals. Serves as a liaison between families and referrals, as needed.
Responds to client needs through flexible scheduling of home visits, including occasional home visits during evening or weekend hours.
Maintains open visit slots and conducts and documents home visits.
Identifies and reports high-risk situations as required per agency and CareStar policy.
Maintains confidentiality as related to patient information. Any disclosures of confidential information made unlawfully outside the proper course of duty will be treated as a serious disciplinary offense.
Minimum Qualifications
Bachelor of Science in Nursing with current RN license, or B.A. degree in another field with current RN License, or Associate of Science in Nursing with State-Level Nursing License.
At least one (1) year of relevant Nursing experience.
Must maintain a current CPR Certification.
Spanish language proficiency, clinical experience related to domestic violence, substance abuse and/or depression, one (1) or more years of experience in child/maternal health, social work, or other related field, and Certified Lactation Educator or Consultant are preferred skills.
